You can speed up your first visit to our clinic by filling out our form for new patients. You can fill out the form on-line and e-mail it to us, or print out the form and bring it with you, filled-out, to your doctor's appointment.

And, remember, when you come to the clinic, please bring your insurance card and your driver's license or another form of indentification. Thank you.
